  • Imaging Reports of Spine

  • Insurance card (if your insurance covers care)

  • Picture Identification Card

  • Any lab work that has been performed

  • Please be sure to dress comfortably, or bring comfortable,
    loose fitting clothes appropriate for examination and treatment

During your first visit, there will be a review your medical history. For diagnostic purposes, there will be an examination of your Spine or any other symptomatic regions. Dr. Emerich will then explain any issues and help you choose options for treatment. We make sure all our new patients feel welcome, taken care of, and safe.
